member ID card includes the member's name, member and group identification numbers, healthcare plan name,
copayment amounts and customer service phone numbers. Members should always have their member ID card with them because it is typically required at every office,
hospital and pharmacy visit. To order a temporary or replacement member ID card,
log in, click the My Account tab.
