Physician Data
Name: the name of the physician. This information is self-reported by the provider on the credentialing application. It is verified every 36 months during re-credentialing.
Specialty: a branch of medicine a physician specializes in such as Family Practice, Internal Medicine or General Surgery. This information is self-reported by the provider on the credentialing application. It is initially verified through the provider's medical school, residence program and fellowship program, the American Medical Association Physician Master Profile and/or the American Osteopathic Association Official Osteopathic Physician Profile. It is re-verified every 36 months during re-credentialing.
Board Certification: a certification by a board that certifies the physician has expertise in a particular specialty, sub-specialty or field. This information is self-reported by the provider on the credentialing application. It is initially verified through the American Board of Medical Specialties, the American Osteopathic Association Official Osteopathic Physician Profile and/or from the appropriate specialty board. It is re-verified every 12 months. If there is no board certification listed on the provider detail page, the provider is not board certified.
Gender: the gender of the physician. This information is self-reported by the provider on the credentialing application. It is verified every 36 months during re-credentialing.
Office Location: the location of the physician's office. This information is self-reported by the provider on the credentialing application. It is verified during the contracting process and with any new medical group affiliations.
Languages Spoken: any languages spoken by the provider and/or clinical staff other than English. This information is self-reported by the provider on the credentialing application. It is not verified. Providers may update languages spoken every 36 months during re-credentialing. If there are no languages listed on the provider detail page, the only language spoken in the provider's office is English.
Acceptance of New Patients: documentation that indicates the physician is accepting new patients into the practice. This information is self-reported by the provider. It is verified during the contracting process and with any new medical group affiliations.
Medical Group Affiliation: the medical group that either employs the physician or counts the physician as its member. This information is self-reported by the physician on the credentialing application. It is verified during the contracting process and with any new affiliations.
Hospital Affiliation: the hospital a physician typically performs specific patient care services in an acute setting. Based on the type of privileges granted by the hospital, the physician may have active, admitting, courtesy or consulting privileges. This information is self-reported by the provider on the credentialing application. It is initially verified with the hospital medical staff office and annually through hospital rosters. Clinical privileges are re-verified every 36 months during re-credentialing.
Hospital Data
Facility Name: the name of the facility. This information is self-reported by the facility on its credentialing application. It is not verified.
Accreditation: the accreditation status the facility has earned from an accreditation organization. This information is self-reported by the facility on its credentialing application. All acceptable accreditations are verified by a copy of the accreditation certificate submitted by the facility at the time of initial credentialing and during re-credentialing. Joint Commission accreditation is also verified through the Joint Commission website. It is verified every 36 months during re-credentialing.
Location: the location of the facility. This information is self-reported by the facility on its credentialing application. It is verified during the contracting process.
