Maternal Health<\/strong><\/p>\n\n\n\n

The Your Healthy Pregnancy<\/strong><\/a> library provides expectant parents educational support on the road to parenthood. Here you\u2019ll find topics ranging from what to ask at the first prenatal visit to what to expect in the delivery room. You can download a pregnancy appointment handbook and the Blue KC Care Management app to connect immediately with the Care Management Team.<\/p>\n\n\n\n

Oncology<\/strong><\/p>\n\n\n\n

The Cancer Care Basics<\/strong><\/a> library walks cancer patients and their families through diagnosis and beyond. It includes a must-read guide, \u201cYou\u2019ve been diagnosed with cancer: Your stay-calm guide to what\u2019s next,\u201d which acknowledges the fear and hope of a cancer diagnosis. And a rich list of topics offers calm, comfort, and control only a trusted resource can provide.<\/p>\n\n\n\n

Blue KC\u2019s Care Management Team \u2013 Your Partner in Health<\/strong><\/p>\n\n\n\n

Our Care Management Team<\/strong><\/a> includes clinical nurses, social workers, patient advocates, and other healthcare professionals. They help members navigate cancer, offer resources for healthy pregnancies, manage chronic medical conditions, provide support and encouragement after a diagnosis, transition after a hospital visit, share inspiration to achieve wellness goals, and answer health questions. The team personalizes a plan based on each member\u2019s unique care needs.<\/p>\n\n\n\n

Behind the scenes, the Care Management Team continuously monitors a dashboard which provides insights about preventive health needs, such as flu shots or annual eye exams, and flags member survey responses related to health, wellness, and nutrition. Our team then works with members to schedule care and offer real-time support.<\/p>\n\n\n\n
