And it\u2019s included in your Blue KC health plan.<\/p>\n\n\n\n<p>The Blue KC Care Management App helps you:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Keep track of doctor appointments and medications<\/li>\n\n\n\n<li>Set medication reminders<\/li>\n\n\n\n<li>Track steps and progress toward health goals<\/li>\n\n\n\n<li>Chat with a live licensed nurse, mental health specialist, or social worker<\/li>\n\n\n\n<li>Manage a chronic condition like diabetes or maintain a healthy pregnancy; the app features more than 70 programs<\/li>\n\n\n\n<li>Explore articles and videos<\/li>\n<\/ul>\n\n\n\n<p><strong>Blue KC\u2019s Care Management Team \u2013 Your Partner in Health<\/strong><\/p>\n\n\n\n<p>The Blue KC Care Management App connects you with our <a href=\"https:\/\/www.bluekc.com\/consumer\/member-benefits\/apps\/wellframe-app.html\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Care Management Team<\/strong><\/a>, which includes clinical nurses, social workers, patient advocates, and other healthcare professionals. They help members navigate cancer, offer resources for healthy pregnancies, manage chronic medical conditions, provide support and encouragement after a diagnosis, transition after a hospital visit, share inspiration to achieve wellness goals, and answer health questions. The team personalizes a plan based on each member\u2019s unique care needs.<\/p>\n\n\n\n<p>Behind the scenes, the Care Management Team continuously monitors a dashboard which provides insights about preventive health needs, such as flu&nbsp;shots or annual eye exams, and flags member survey responses related&nbsp;to health, wellness, and nutrition.
