They are trained to look after your overall wellness, manage chronic illnesses, and keep you on track with screenings that can help prevent serious conditions. By providing preventive care, a PCP can help you manage everything from diabetes and high blood pressure to arthritis and anxiety \u2013 which can improve your quality of life.\u00a0\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A PCP can also coordinate your referrals with specialists and any other providers you might need, as well as order necessary labs. This continuity of care is designed to make it easier for you to navigate your health journey and help you focus on what matters most \u2013 getting healthy and staying heathy.&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Visit your PCP and earn rewards<\/strong>&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When you visit your PCP for your annual preventive before December 31, 2026, you can earn a $25 gift card through the ACA Rewards program. Earn an additional $25 gift card by completing the Health Risk Assessment*.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It takes just three steps to earn rewards:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Register on the ACA Rewards portal at\u00a0<a href=\"https:\/\/bluekcaca.healthmine.com\/login\"><strong>BlueKC.com\/ACARewards<\/strong><\/a>.<\/li>\n\n\n\n<li>Schedule an annual preventive visit with:\n<ul class=\"wp-block-list\">\n<li>Primary Care Provider (PCP) \u2013 Visit your in-network PCP or find one at MyBlueKC.com &gt; Find Care &gt; Doctors and Specialists &gt; Search Doctors. In the search bar enter primary care or family medicine to find a PCP in your area.<\/li>\n\n\n\n<li>Spira Care provider \u2013 Available to members enrolled in plans with exclusive access to Spira Care Centers. Appointments are available by calling 913-297-7472 or at <a href=\"https:\/\/www.spiracare.com\" target=\"_blank\" rel=\"noreferrer noopener\">SpiraCare.com<\/a>.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Complete the HRAat <a href=\"https:\/\/www.BlueKC.com\/ACARewards\">BlueKC.com\/ACARewards<\/a>.\n<ul class=\"wp-block-list\">\n<li>Once logged in, click Health Risk Assessment.
