Small group applications for 2023 are now available. For new business you will find employee and group applications for ACA, fully insured, ASO, and ChamberCHOICE.
Access them on your agent portal.
- Log into agents.bluekc.com
- Click Resources in the left column
- Click Plan Documents/Forms
- Click Small Group tab
- Choose the group size and click the applicable link
Please delete older versions of Blue KC applications you may have saved to your files. If you have questions about the applications, contact your Blue KC representative.
