Blue Medicare Advantage ID cards display a member’s primary care provider (PCP) name in support of our continuity of care efforts. Should a member visit the ER or have a hospital stay, the PCP can easily be identified and contacted.
The name that appears on the card is based on the provider of record from claims history; it represents the doctor a member sees for routine services.
If a member wishes to have a different PCP printed on the card, they simply need to call customer service to have it changed.
