You can now submit a census enrollment for new ACA or ancillary-only small group policies.
Please complete the following documents.
- Employer/Group Application
- Employer Size Survey (medical only)
- Census Template
- Additional documentation is still required for groups with only two full-time employees (W-2/payroll/Quarterly Wage & Tax Statement)
