New Spira Care program aims to optimize patient medications, health outcomes

Spira Care has launched the Comprehensive Medication Management (CMM) program, an innovative, patient-centered approach to assessing patient medications to help ensure they are appropriate, effective, safe given other health conditions and medications, and accessible.

The program’s initial focus is on patients with complex conditions who are transitioning out of the hospital. Soon, it will expand to patients through their Spira Care providers.

Helping to ensure Spira Care patients get the most benefit from advances in pharmacology is a critical component of improving their health. CMM is an innovative connector between primary care, hospitals, partner health systems, specialty healthcare providers, pharmacy, and health insurance. It supports improved medication-related outcomes and offers increased attention to medications and their impacts on a patient’s daily life.  

CMM embodies the Spira Care model of Advanced Primary Care, offering a personalized, patient-centered, simplified healthcare experience. The program delivers individualized care plans to help patients achieve the intended goals of each medication with appropriate follow-up to monitor outcomes. 

Delivered by Spira Care’s own ambulatory pharmacist

Spira Care’s ambulatory care pharmacist, Bailey Scism, PharmD, BCACP, delivers the CMM program and works in collaboration with patients and other healthcare providers.  

Dr. Scism is embedded in the primary care setting to support and empower patients to proactively manage their health goals, diagnoses, and prescriptions. She works side-by-side with Spira Care physicians and advanced practice providers in the transitions of patients who have been hospitalized. She also helps ensure prescriptions are safe, effective, and affordable for each patient’s unique needs and insurance formulary interpretation.  

Through CMM, Dr. Scism applies her pharmacotherapeutic expertise in a collaborative process with Spira Care Teams, allowing physicians to be more efficient, see more patients, and spend more time providing medical care.

*Blue KC members enrolled in plans (without a Health Savings Account) with exclusive access to Spira Care Centers will have no costs for any procedure provided at a Spira Care Center. Blue KC members enrolled in plans (with a Health Savings Account or HSA Eligible) with exclusive access to Spira Care Centers will have an affordable charge for services. Preventive services are covered at 100 percent. Health and Welfare Funds who contract with Blue KC to offer exclusive access to Spira Care Centers provide their non-Medicare eligible participants access to appointments and services at Spira Care Centers at no out-of-pocket cost.
