Keeping up with healthy habits can be challenging, but the Blue KC Care Management app* helps make it easier with a variety of features and benefits.

When you download the Blue KC Care Management app, you have access to a selection of exclusive perks and discounts. Explore offers on goods and services including groceries, dining, health and wellness, entertainment, and more.

Do you have travel coming up? Browse discounts from brands like VRBO, Avis, and more. 

Looking for healthy meal delivery? We have you covered with offers from brands including DoorDash and Factor Meals.

To get started, simply download the Blue KC Care Management app in the App Store or Google Play, or scan the QR code below. Use access code kcbluewelcome when prompted. To explore the perks and discounts, create an account and log in. Then tap “Resources” followed by “Your Perks.”

How a simple chat can help set you on the path to better health

One of the best parts of the Care Management app is the chat. After you sign up, you’ll get a welcome message from a real member of the Blue KC Care Team*. Yes, an actual person, not a bot or A.I.

The local Blue KC Care Management team includes registered nurses, social workers, and Community Health Workers. They’ll connect with you directly through the app—real people helping real members navigate their daily healthcare needs. They’re also there to guide you in setting up helpful app features like:

  • Health trackers: Members can set up trackers to support their health goals. For example, if you have diabetes, you can regularly track your glucose. The Care Management team can spot patterns and help catch potential issues early. Then, they can help you to book an appointment with your doctor to address the issue.
  • Personalized, app-based programs tailored to your chronic condition or health goals: Based on your care profile, the Blue KC Care Team can connect you with support and share easy-to-follow articles and videos to help you on your health journey.
  • Access to community support: Chat with the Blue KC Care Team for support beyond medical care. They can connect you to resources for things like food insecurity, transportation, financial assistance, and more.

With five key features, The Blue KC Care Management app offers personalized support to help you reach your health goals. The app includes:

  1. Chat: Reach your Blue KC Care Team* of registered nurses, social workers, and Community Health Workers ready to guide and support your care.
  2. Set reminders to stay on track: Whether it’s appointments, drinking water, or getting active, every step moves you closer to your goals.
  3. Manage medications: Track your medications, set helpful reminders, and read tips on how to talk with your doctor or pharmacist.
  4. Unlock perks on everyday favorites: Enjoy savings on entertainment (SiriusXM, Apple TV), food (HomeChef, DoorDash), and travel (gas, VRBO, car rentals).
  5. Tips: Find guides and articles to help you prep for checkups and ask the right questions to advocate for your health.

Download the Blue KC Care Management App Scan the QR code below or download the app in the App Store or Google Play. When prompted, use access code kcbluewelcome. Blue KC members can also access Blue KC Care Management through the MyBlueKC app under the Health and Wellness tab.
