Get support for managing diabetes from the Blue KC Care Management Team

Turn to Blue KC for support managing your diabetes. Our Care Management Team includes registered nurses, social workers, and community health workers you can chat with securely through the Blue KC Care Management app.

With the Blue KC Care Management app you can also:

  • Set appointment and medication reminders
  • Receive daily health tasks and track your progress
  • Explore clinically approved resources

Work with a Diabetes Care Specialist at a Spira Care Center2

If you are a Blue KC member with exclusive access to Spira Care Centers, talk to your Spira Care provider about working with a Diabetes Care Specialist. Diabetes Care Specialists Amy Kapple, RDN, LD and Janice Monachino, RN understand the challenges of managing diabetes and can help develop a diabetes management plan that works for you. They’ll also:

  • Help you understand your diabetes or prediabetes diagnosis  
  • Partner with you and your primary care provider to develop a care plan that fits your schedule and lifestyle  
  • Support and advocate for you  
  • Help you achieve your goals and feel your best
