The “Checklist” tool turns your health needs into a daily checklist. It helps you take key steps toward improved health and wellness.

What is Blue KC Care Management?

Blue KC Care Management is a team of registered nurses and community health workers trained to help you navigate your care and connect you with resources. The Blue KC Care Management team does not provide treatment or medical advice and is available during normal business hours. Care management, delivered over the phone or through the Blue KC Care Management app, is included in your plan benefits at no additional cost.* Participation is optional and won’t affect your benefits.

What does the Blue KC Care Management app cost?

The Care Management app is included in your Blue KC plan benefits at no additional cost* to you.

What is my access code?

The access code you receive is required to complete your registration. It will be a series of letters and/or numbers provided by Blue KC that you’ll use to register for the Blue KC Care Management app.

Why isn’t my access code working?

Make sure you’re typing it in correctly. It should have all lowercase letters and no spaces. It may include numbers. If you need assistance, call the Wellframe team at 1-844-452-4085 or send an email to help@wellframe.com.

How much data does the Blue KC Care Management app use?

The app uses minimal data to work properly on your smartphone or tablet. It’s unlikely to cause you to go over your data limit, especially if you’re using the app on a WiFi network.

What is Wellframe?

Wellframe is an independent company providing technology that enables person-to-person connections and improves the care management experience.
