Blue KC’s Care Management Team – Your Partner in Health
The Blue KC Care Management App connects you with our Care Management Team, which includes clinical nurses, social workers, patient advocates, and other healthcare professionals. They help members navigate cancer, offer resources for healthy pregnancies, manage chronic medical conditions, provide support and encouragement after a diagnosis, transition after a hospital visit, share inspiration to achieve wellness goals, and answer health questions. The team personalizes a plan based on each member’s unique care needs.
Behind the scenes, the Care Management Team continuously monitors a dashboard which provides insights about preventive health needs, such as flu shots or annual eye exams, and flags member survey responses related to health, wellness, and nutrition. Our team then works with members to schedule care and offer real-time support.
